June Driver, Eva Blackburn and Annie Midgley died in hospital and were thought to have been poisoned.

A nurse was charged with their murders but she accidentally overdosed on anti-depressants and died before her trial. At the nurses inquest, it was ruled that her death was accidental and not suicide.

An inquiry published in 2010 stated that it was unlikely that the nurse had set out to deliberately poisoned the three women and found that there was a catalogue of systemic failures in the way she was allowed to carry out her work. The inquiry stated that the decision to prosecute the nurse was the right one but that when they went through the clinical records that she had kept, the charts and notes, it was found that her records were entirely open and that the events that had occurred were instead the result of a combination of individual and system failures.

June Driver, 67, died in July 2000. She was being treated for an infection after a hip replacement.

Eva Blackburn, 75, died in November 2001.

Annie Midgley, 96, died in July 2002. She had been admitted to hospital with abdominal pain.

The nurse had also been charged with attempting to murder another man aged 42 in June 2002 and had been charged with 13 counts of unlawfully administering poison to 12 other patients.
